3D Printed Total Talus Replacement after Free Vascularized Medial Femoral Condyle Osteocutaneous Flap for Avascular Necrosis of the Talus Leads to Poor Clinical Outcomes

This study examined the outcomes of patients with avascular necrosis (AVN) of the talus who underwent a 3D-printed total talus replacement (TTR), either as a primary procedure or as a secondary procedure following failure of a free vascularized medial femoral condyle (MFC) flap. AVN of the talus is a challenging condition to diagnose and manage, and traditional treatment options include joint sparing and joint sacrificing procedures. The use of 3D-printed TTR has become more popular as a way to preserve joint motion in patients with talus pathology. <br /><br />The study analyzed the outcomes of two patients who underwent secondary 3D-printed TTR after failure of an MFC flap, and compared them to a cohort of patients who underwent primary 3D-printed TTR. The assessment included ankle range of motion, functional outcome scores, pain scores, and radiographic imaging, with a minimum follow-up period of 12 months.<br /><br />The results showed that the patients who underwent 3D-printed TTR after MFC flap failure had poor functional outcomes compared to those who had primary 3D-printed TTR. The study concluded that the use of 3D-printed TTR after talar revascularization with MFC grafts may not lead to satisfactory clinical outcomes for patients with AVN of the talus.<br /><br />This study highlights the challenges in treating AVN of the talus and suggests that 3D-printed TTR may not be an effective solution after MFC flap failure. Further research is needed to explore alternative treatment options for patients with talus pathology.
