Clinical and Radiological Results of a Human Bone Graft Screw in Tarsometatarsal II/+III Arthrodesis

This study evaluated the clinical and radiological outcomes of Lisfranc arthrodesis using a novel method, arthrodesis with a bone screw that integrates into the recipient bone. Twenty-three patients (25 feet) were included in the study, and clinical and radiologic outcomes were evaluated. Clinical parameters such as AOFAS forefoot score, Foot Function Index (FFI), Foot and Ankle Outcome Score (FAOS), and Visual analogue scale (VAS) for pain were compared with preoperative parameters. Radiologic evaluation included assessing fusion rate and integration of the Shark screw® into the recipient bone. The average follow-up time was 14.6 months.<br /><br />The results showed significant improvement in clinical parameters postoperatively. The AOFAS, FAOS, FFI, and VAS pain scores all displayed improvement. Radiologically, there were no cases of pseudarthrosis, and complete integration of the bone screw was seen in all patients after 12 months. However, there were some complications observed, including hypesthesia of the superficial peroneal nerve, secondary extensor tendon rupture, and superficial infection.<br /><br />The study discussed that overall, the results were consistent with the literature, but no previous study had investigated the results with this new device. It was also noted that other deformities and joints were treated in some patients, and the AOFAS score used for clinical assessment may not be sufficiently validated.<br /><br />In conclusion, the study demonstrated that Lisfranc arthrodesis with the Shark screw® resulted in a significant improvement in clinical scores. The screw proved to be a valid solution for fusion of the lesser Lisfranc joints without the need for metal removal.
