Impact of Pre-Operative PROMIS Depression Scores on Total Ankle Replacement Outcomes

The study aimed to evaluate the impact of pre-operative PROMIS depression scores on total ankle replacement (TAR) outcomes. The Patient-Reported Outcomes Measurement Information System (PROMIS) was used to assess TAR outcomes. Previous studies have shown that pre-operative factors like stress and depression may influence the success of TAR, but there is limited literature on this topic. <br /><br />The research included a retrospective analysis of patients enrolled in the Orthopaedic Foot and Ankle Outcomes Research (OFAR) network. Patients were grouped as "no/low depression" if they scored below 60 on the PROMIS depression scale, and "high depression" if they scored 60 or higher. Mann-Whitney U tests were used to assess significance. <br /><br />Both groups showed significant improvement in PROMIS physical function and PROMIS pain interference at 12 months. The high depression group had a significantly higher baseline PROMIS pain interference compared to the no/low depression group, but there was no significant difference between the two groups at 12 months. There was a significant difference in PROMIS depression scores between the high depression and no/low depression groups pre-operatively, but no significant difference at 12 months. <br /><br />The findings suggest that patients with high pre-operative PROMIS depression scores may have worse baseline physical function and pain interference compared to other patients, but they see greater overall improvement after total ankle arthroplasty. <br /><br />The study included 36 patients who received a total ankle replacement, with 28 categorized as no/low depression and 8 categorized as high depression. The data was made available by the Orthopaedic Foot and Ankle Outcomes Research network, funded by the American Orthopaedic Foot and Ankle Society.
