Isolated Talonavicular Arthrodesis as Treatment for Flexible Progressive Collapsing Foot Deformity: A Case Series

This study evaluated the use of isolated talonavicular arthrodesis as a treatment for flexible progressive collapsing flatfoot deformity (PCFD). The study included 49 patients (59 feet) with PCFD who underwent the isolated TN fusion procedure. The patients' preoperative and postoperative radiographs were analyzed, measuring variables such as the Meary angle, calcaneal pitch, TN coverage angle, and Simmon angle. Clinical outcomes were also assessed using the Foot and Ankle Ability Measure (FAAM) and the Veterans-Rand 12 Item Health Survey (VR-12).<br /><br />The results showed that isolated TN arthrodesis is a viable option for the treatment of PCFD. The average age of the patients was 61, and the average follow-up period was 19.23 months. The postoperative FAAM scores were significantly improved compared to the preoperative scores (p < 0.001). However, there were no significant changes in the VR-12 physical and mental scores.<br /><br />The study concluded that isolated talonavicular arthrodesis can effectively address the unhealthy TN joint and correct deformities at the CORA (center of rotation of angulation). The procedure offers early improvement in FAAM scores and shows good support in the literature. The postoperative radiographic measurements also demonstrated improved alignment, with significant reductions in the Meary angle and improvements in the calcaneal pitch and TN coverage angle.<br /><br />Overall, the study suggests that isolated TN arthrodesis can provide a successful treatment option for flexible PCFD, offering lasting correction and improved functional outcomes.
