The study aimed to quantify the changes that occur in the peritalar configuration following ankle arthrodesis for varus-type osteoarthritis of the ankle (VAOA). A total of 14 patients who underwent arthrodesis for VAOA were included in the study. The results showed that there were significant improvements in foot alignment after surgery, with a decrease in the foot ankle offset (FAO) value. Distance mapping analysis revealed that there were shifts in the joint center points in the tibiotalar, subtalar, talonavicular, and calcaneocuboid joints after surgery. The largest shift vectors were observed in the talonavicular joint, followed by the tibiotalar and subtalar joints. There was a strong correlation between the FAO value and the medial-lateral shift of the subtalar joint. The study concluded that peritalar changes do exist after ankle arthrodesis for VAOA, particularly in the talonavicular joint. The flexibility of the talonavicular joint is important when considering arthrodesis for VAOA, and alternative treatments such as total ankle replacement or combined realignment osteotomies should be considered when the subtalar and talonavicular complex is stiff. The study also suggested that the measurement of the joint center shift vector could be a useful tool to investigate peri-operative joint changes based on weight bearing CT data. However, it is important to note that the study had limitations due to the small number of cases and the short postoperative follow-up period.
