I'd like to thank AOFAS for the opportunity to present our work entitled The Effect of Chilled vs. Room Temperature Irrigation on Thermal Energy Dissipation During Minimally Invasive Chalconeal Osteotomy. These are our disclosures. We did receive a grant from NOVASTEP for this project. Minimally invasive chalconeal osteotomy has grown in popularity in recent years to address hindfoot deformity. A potential complication is thermal bone necrosis secondary to heat generation. The effect of this irrigation on reducing heat accumulation during MIS chalconeal osteotomy remains an understudied area. The purpose of this study was to evaluate the effect of cooled vs. room temperature irrigation on thermal energy dissipation on a temperature curve during a chalconeal osteotomy using a Shannon burr. Fourteen catavaric limbs at room temperature or chilled irrigation were randomized. So seven limbs received a cooled temperature irrigation and seven limbs received room temperature irrigation. Two thermocouple probes were inserted five millimeters away from the plane of the osteotomy on the proximal and distal sides. A three by thirty millimeter Shannon burr was used and a new burr was used for each osteotomy. The burr was run continuously with continuous irrigation utilizing either room temperature at sixty eight degrees Fahrenheit or chilled irrigation at thirty seven degrees Fahrenheit until the osteotomy was completed. All osteotomies were performed over the course of sixty seconds. The temperature was recorded at zero, fifteen, thirty, forty five, and sixty seconds for the two groups and used as a measure of thermal energy accumulation. This is a picture of our setup demonstrating the use of the Shannon burr, the position of the two thermocouple probes, the position of the saline tubing. Both room temperature and chilled irrigation were effective in minimizing temperature change. On the proximal side an overall mean increase of two point five degrees with room temperature irrigation and a mean decrease of one degree with chilled irrigation were observed at the sixty second interval. On the distal side there was a mean increase of one point three degrees with room temperature irrigation and a mean increase of zero point five degrees with chilled irrigation. This is a bar graph demonstrating the decrease in temperature at the forty five and sixty degree intervals with the use of chilled irrigation. And then lastly a tabular format of our data demonstrating decrease in temperature observed with the use of chilled irrigation versus room temperature irrigation. In conclusion, for the duration of a MIS calcaneal osteotomy, both continuous and room temperature irrigation can reduce thermal energy accumulation. The use of chilled irrigation is more effective in reducing temperature change during the osteotomy. And this should be considered in osteotomies of denser bone that may take longer to perform and generate higher temperatures.
