The Position and Morphology of the Peroneus Longus Tubercle in Hallux Valgus: A Weight-Bearing CT Assessment

This study aimed to compare the position and morphology of the peroneus longus tubercle (PLT) in feet with and without hallux valgus (HV) using weight-bearing CT (WBCT). The primary outcome found significant differences in the T-F distance, indicating a lower medial column in HV. There was also a moderate correlation between the intermetatarsal angle (IMA) and T-F distance, suggesting depression of the first ray with increasing IMA. The bisecting angle was lower in the HV group, demonstrating pronation of the PLT in relation to the floor. The T-MT angle suggested pronation occurred in the first metatarsal to a larger degree than the rest of the forefoot. However, there were no significant differences in the cross-sectional areas of the PLT between groups. <br /><br />The study also proposed three new measurements for assessing the PLT with good intra- and inter-observer reliability. The conclusions indicated that the PLT is pronated and lower in individuals with HV. Further research is needed to determine whether the altered position of the PLT is a consequence of HV or a predisposing cause. <br /><br />Overall, this study provides new insights into the position and morphology of the PLT in HV and suggests the potential role of the peroneus longus tendon in the dynamic stabilization of the first ray. This information may contribute to improved understanding and management of HV.
