Pdf Summary
This case report discusses a rare injury of a traumatic great toe MCL avulsion in a professional surfer. The patient, a 23-year-old male competitive surfer, experienced left great toe pain after hyperdorsiflexion while surfing. The goal was to treat the injury surgically in order to allow for a rapid return to professional surfing.

The surgical approach involved a mid-axial approach to the hallux MTPJ. The MCL was mobilized and repaired using suture anchors and suture tape augmentation. The repair resulted in good outcomes, including a rapid return to sport. The patient was cleared for return to professional surfing just 4 weeks post-operation.

This surgical technique of using suture anchors and suture tape augmentation for hallux MCL repair has not been reported in a weightbearing joint like the hallux MPJ before. However, the outcomes of this case suggest that it is a viable surgical option for hallux MCL rupture with good results and fast return to activity.

The post-operative progress showed good healing, active range of motion, and weight-bearing as tolerated in a CAM boot. By 3 months post-operation, the patient had full activity, without any deformity recurrence or pain, and had equal range of motion compared to the contralateral joint. The patient plans to participate in a national surfing competition.

In conclusion, this case report suggests that surgical treatment of traumatic great toe MCL avulsion using hallux MCL repair with suture anchors and suture tape augmentation can lead to a rapid return to sport. The technique provides good outcomes and allows for an easy rehabilitation process.
