Pdf Summary
Chronic ankle instability is a condition that can affect individuals of all ages, causing pain and limited mobility. It primarily affects the lateral ankle and can range in severity. Non-surgical treatment options should be attempted first, but if unsuccessful, surgical management is highly effective. The anatomy of the ankle includes both dynamic and static stabilizers, such as ligaments and muscles, that work together to provide stability. The main factors contributing to chronic ankle instability are mechanical and functional, including laxity in the joint, changes in the synovial fluid, and impairments in proprioception and neuromuscular control. The clinical presentation of chronic ankle instability includes persistent pain, recurring episodes of ankle giving way or spraining, and a lack of confidence in walking on uneven ground. Physical examination may reveal difficulty with single leg standing and apprehension with lateral movements. Inversion stress radiographs and magnetic resonance imaging can be useful in diagnosing chronic ankle instability and associated injuries. Treatment options include a structured functional rehabilitation program, external splinting, and surgical repair. Anatomic repair aims to restore normal ligament anatomy, while non-anatomic repair involves supplementation or reconstruction of the ligaments. Surgical reconstruction has shown to be successful for those who have failed non-operative treatment. Future research is looking into alternative techniques and the use of ankle arthroscopy for ligament repair and associated intraarticular pathology. Overall, chronic ankle instability can greatly impact an individual's quality of life, but there are effective treatment options available.
