Hallux rigidus is a degenerative condition of the joint between the 1st metatarsal head and the base of the proximal phalanx of the hallux. It causes pain and limited range of motion in the foot. The joint allows for various planes of motion, including plantarflexion, dorsiflexion, abduction/adduction, and pronation/supination. Causes of hallux rigidus include previous trauma, chondral injuries, ligamentous injuries, inflammatory arthritis, and chronic deformity. Diagnosis is typically done through plain radiographs, which can show joint space narrowing, osteophytes, subchondral cyst formation, and sclerosis. Conservative treatment options include medication, orthotic devices, and shoe wear modifications. Surgical treatments range from joint debridement and cheilectomy to correctional osteotomies and arthroplasty. Interposition arthroplasty and MTP hemi-arthroplasty have shown promising outcomes. In advanced cases, arthrodesis of the hallux MTP joint may be considered. Both conservative and surgical treatments can provide pain relief when used appropriately.
